The Sweepstakes ("Sweepstakes") begins on Thursday, April 28, 2016 at 8:00 a.m. Pacific Daylight Saving Time ("PDT") and ends on Sunday, May 8, 2016 at 11:59 p.m. PDT (the "Sweepstakes Entry Period"). Sweepstakes is sponsored by Football Northwest LLC d/b/a the Seattle Seahawks ("Seahawks" or "Sponsor"). ELIGIBILITY: Sweepstakes open to all legal residents of Washington, who, as of the time of entry are eighteen (18) years or older ("Entrant"), except employees, officers, directors, agents and representatives of the Seahawks or CenturyTel Service Group, LLC ("CenturyLink"), its respective parent corporation, affiliates, subsidiaries, franchisees, advertising and promotion agencies, suppliers, distributors, and members of the immediate family and those living in the same household of each are not eligible to enter. All mail in entries must be postmarked on or before April 28, 2015 at 4:00 p.m. PDT and received no later than May 6, 2016 at 4:00 p.m. PDT. Mechanically reproduced or photocopied entries are void. On-line entries will be declared made by the authorized account holder of the e-mail address submitted at time of entry. "Authorized account holder" is defined as the natural person who is assigned to an e-mail address by an Internet access provider, online service provider or other organization (e.g., business, educational, institution, etc.) responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. CenturyLink is not responsible for late, lost, misdirected, mis-delivered, incomplete, incorrect, inaccurate or unintelligible entries, messages, typographical errors, unavailable network connections, failed, incomplete, garbled, or delayed computer transmissions, on-line failures, hardware, software, servers, providers, equipment, human error or other technical malfunctions or disturbances or any other communications failures or circumstances affecting, disrupting or corrupting the Sweepstakes in any manner. If the winning entry is an on-line entry, the entrant may be required to provide Sponsor with confirmation that he/she is the authorized account holder of the listed email address (if any). Sweepstakes Entities are not responsible for any charges from the wireless phone provider made to the entrant. Entrants must have a text messaging two-way capable handset and digital service in order to enter the Sweepstakes using the text messaging method. Entrants will be sent a text message that confirms entry into the Sweepstakes. Entrants entering by text will be charged standard text messaging rates for text messages sent and received from their handset according to the terms and conditions in their wireless calling plan. PRIZES: There will be one (1) grand prize awarded. The grand prize will consist of a winner and one guest attending a practice visit during June 2016 Mini-Camps, tour of the Virginia Mason Athletic Center, lunch with 2016 Seahawks Draft pick(s), and one custom Seahawks jersey. Grand prize is approximately $160 in value (value of jersey only). Prize does not include any additional expenses to redeem Prize including, but not limited to, travel, lodging, incidentals, etc. Taxes on prizes are the sole responsibility of winners. DRAWING: One Winner will be selected from among all eligible entries received to receive One Grand Prize. One (1) random drawing to be conducted on Friday, May 6, 2016 at 5:00 p.m.[the sweepstakes ends at 11:59] PDT (after final US postal mail delivery). The random drawing will be conducted under the direct supervision of the Seahawks. Winner will be notified by phone on or around Monday May 9, 2016. Odds of winning are determined by the total number of eligible entries received. PRIZE CLAIM: Noncompliance by a potential winner and/or his or her guest will result in disqualification and an alternate winner will be selected. Any prize notification returned as undeliverable will result in disqualification and an alternate winner will be selected. Claimed prizes will be awarded. Winner and guest must complete, execute and return an affidavit confirming the winner's identification and eligibility including a Liability and Publicity Release. Guest must complete and execute a release form prior to participation. If such documents are not executed at the time of Sweepstakes or if any winner is found to be ineligible or not in compliance with these official rules, disqualification may result. Taxation. Winners are solely responsible for any and all federal, state, and local income taxes associated with his or her Prize. Sponsors are not responsible for and will not pay or in any way compensate Winners for taxes associated with winning a Prize in this Sweepstakes. The Winners may be required to provide certain tax related documents to Sponsor before receiving a prize. If required by law, Sponsor will issue Form 1099-MISC to Winners reporting the taxable retail value of his or her Prize. The Winner should consult his or her tax advisor about the tax implications of winning a Prize. GENERAL: The Winner and guest grants Sponsor and its franchisees, CenturyLink and the Seahawks full permission and authority to use, re-use and license the use of their voice, name, photograph or likeness in any promotional activity related to their being a sweepstakes winner, except where prohibited by law, and further grants Sponsor and CenturyLink the right to use contestant contact information for its own use to contact contestants about other Sponsor or CenturyLink programs and that they are entitled to no further compensation from Sponsor, CenturyLink or the Seahawks. Sponsor and its agency are not responsible or liable for lost, late, incomplete, misdirected, not received, stolen, mutilated or illegible entries or mail. By entering this drawing, Entrants accept and agree to be bound by these rules and the decisions of the judges, which are final, binding and conclusive on all matters relative to this Sweepstakes. Seahawks reserves the right to disqualify entrants who fail to follow these official rules and regulations or who make any misrepresentations relative to the Sweepstakes, drawing and redemption of prizes. Winners may not substitute or transfer prizes. Sponsor reserves the right to substitute prize of equal or greater value should featured prizes become unavailable. Seahawks, CenturyLink and Sponsor, their parents, subsidiaries, affiliates, franchisees and agents are not responsible for any negligence, claims, liability, injury or property loss or other damages of the winner (and their guest) arising from, or in connection with the prizes awarded or participation in this Sweepstakes. Under no circumstances will Winner or guest be permitted to obtain awards for, and participant hereby waives all rights to claim punitive, incidental, consequential or any other damages, other than for actual out-of-pocket expenses; and that all causes of action arising out of or connected with this Sweepstakes, or any prizes awarded, shall be resolved individually, without resort to any form class action.
