[russian/holter.htm] «World Holter» – long-term ECG monitoring system Holter ECG monitoring - is the widespread method of functional diagnostics used in clinical practice to reveal heart rate disorders and ischemic changes of ECG , for clarification of their mechanisms, as well as for selection and control of antiarrhythmic therapy . The method provides for continuous (day-long or more) registering of ECG of a patient, living practically normal life, with the help of portable recorder . Then within several minutes ECG is loaded into PC where it is automatically analysed by special software . The « World Holter » system is a high-performance multi-channel Holter analyser ideal for use in modern clinics. Combination of broad functionality and well considered interface makes the system equipped with everything required for accurate and fast ECG analysis summarised in the form of printed report. The system uses state-of-the-art digital recorder Lifecard CF by Del Mar Reynolds Medical company, which has the following features: continuous ECG recording during 7 days; detection of the artificial cardiac pacemaker impulses; built-in monitor for display of ECG and pacemaker when connected; only 3 electrodes for recording of 3 ECG channels; 12-bit data resolution; power from one AAA battery. «World Holter» software performs the following tasks: Loading ECG data from recorder Alongside with top of the range digital recorder Lifecard CF , the « World Holter » system supports entering ECG data from the other digital and analogue recorders by Del Mar Reynolds Medical, including those having standard recording speed on tape of 1mm/sec. The length of the recording that the system is capable to load is limited only by recorders' capabilities. One can preview the ECG during loading for cursory examination. Automatic ECG analysis Fully automatic ECG analysis detects all QRS complexes (as well as their specific points Q, R, S and T), determines their type (N, V, SV, X) as well as identifies arhythmia events . The system allows: to detect heart rate and conduction disorders : pauses ( asystole ) pairs bradycardia bigeminy trigeminy V rhythm ventricular tachycardia ventricular bradycardia sinus tachycardia SV rhythm supraventricular tachycardia supraventricular bradycardia to analyse changes of ST segment and to detect myocardial ischemia episodes; to calculate QT intervals and dispersions ; to analyse heart rate variability ; to analyse operational mode of artificial cardiac pacemaker . Review and editing of ECG analysis results ECG analysis results are displayed in the form of tables and graphs which can be edited. Original methodology of data display allows for quick and easy review. This is achieved through " relevant selection " mechanism - a unique technology that allows comparison of any tabular or graphical data with the corresponding original ECG elements. For example, selection of a histogram bar showing certain R-R intervals range on R-R intervals distribution histogram will result in marking on the ECG only QRS complexes with R-R interval that fits the one on histogram. At the same time there is a possibility of quick review and editing of ECG sections relevant to the selection. In addition to the option of working with complete ECG recording, there is an option allowing to split it into several time periods (e.g. day, night, before and after taking medicine, etc.) and get individual results for each period. Elements of report of one type (table or graph) for various time periods are printed in sequence. This structure of the report facilitates considerably evaluation of patient status dynamics. Making report Report subsystem allows to get print-out of the analysis results, to save report on disk and to email it. Sections of the report as a whole as well as its separate elements are generated with the help of report format mechanism, parameters of which can be set up by user and stored in the system. « World Holter » system is flexible and adjustable product capable to meet requirements of both general practitioner and medical researcher. In addition to standard parameters the system allows to create user's set of parameters for identification of criteria for arrhythmia detection , QT calculation , criteria for elevation/depression of ST segment . There is also an option of exporting analysis results into widely used formats of databases for subsequent statistical processing. In addition to stand-alone operation « World Holter » software can operate in full integration with «CardioNavigator» Cardiology Information Management System by Del Mar Reynolds Medical, which makes it convenient for use in clinics. This system co-ordinates and manages a diversity of noninvasive investigations as well as stores the results. «World Holter» software operates on several European languages and has advanced help subsystem.
