The following documents should be presented along with the petition, this is so called dossier: 1. Adoption Home Study with Agency license & Social Worker license 2. Letter of approval for adoption from immigration 3. Statement of employment and income from previous year 4. Statement of health from physician for prospective parents 5. Copy of marriage certificate 6. Copy of passports of prospective parents 7. Letter of clearance from police 8. Letter of obligation that adopted child/children will retain Ukrainian citizenship until 18 years of age and that he/she/they will be registered with the Ukrainian embassy until 18 years of age. 9.
