[main.php] Hernia treatment Inguinal and ventral hernias History External hernia of abdomen is one of the most widespread surgical disease (3-7% population). Among external hernias of abdomen the inguinal hernia is more often widespread. External hernias of abdomen can be acquired and postoperative (after the operation on the organs of abdomen cavity). The importance of problem of surgical treatment of hernias is explained by great number of relapses (repeated appearance) without a tendency to the decreasing from 15 to 20%. The first announcement about polypropylene appeared in USA in 1958. F.Usher with his co-authors reported about good results, reparation of hernias by the use of tissue network from polypropylene. In 1962 the technology of knitted polypropylene network was developed which one can stretch along both axises, adequately closing an abdomen defect. This property of network especially needed for the correction of large defects of abdomen network. In 1989 American surgeon Lichtenstein offered to perform the plasty of posterior wall of inguinal canal (in inguinal, femoral hernias) by the polypropylene network which enough quickly germinates by own tissues and becomes the solid wall of inguinal canel. Advantages The use of polypropylene network in treatment of external hernias of abdomen: 1. Intention of tissue of hernia defect is absent; 2. Fast germination of network by own tissue allows to decrease the frequency of relapses of hernias ( less 1%). Indications 1. Ventral hernias (acquired, postoperative). 2. Slanting also straight inguinal hernias. 3. Femoral hernias. 4. Postoperative, relapse (repeated) hernias. Contra-indications 1. Violation of blood coagulability. 2. Presence of chronic and acute infections. 3. Any somatic diseases (diseases of internal organs in the critical phase). 4. Strangulated hernias. Preoperative preparation 1. Hospitalization in the clinic at the day before the operation. 2. Hunger (not less than 6 hours before the operation). 3. Cleaning enema at the day before the operation. 4. Medicamental therapy about indications. Operation Evolving of hernia sac and dissection it. Plasty of hernia hilus (i.e. hernia defect) with assistance of polypropylene network which has to be at corresponding sizes for exact hernia defect with fixation of polypropylene network with surrounding tissues (inguinal ligament, anterior abdomen wall). The duration of the operation is from 1 to 3 hours. Postoperative period 1. Early activity of patients (in 6-8 hours after the operation). 2. Nutrition in 24 hours after the operation. 3. Painful syndrome is not significant. 4. Be in the clinic from 2 to 5 days. 5. The removal of sutures on the 7-9 days. 6. Restriction of physical loading for 2 months; in consequence the exercises without the use of dumb-bells, weight. In 6 months after the operation it is possible an unlimited physical loading. Possible postoperative complications 1. Hematoma, seroma in the area of postoperative wound. 2. Edema of pubis, scrotum. 3. Injury of spermatic cord. 4. Extension of orchiepididymitis (inflammation of testicle and spermatic cord). 5. Suppuration of postoperative wound.
