[main.php] Who is a plastic surgeon and whom consider a qualified doctor? The plastic surgeon is not a single doctor studying an aesthetic surgery. Such operations are quite natural, if they are performed by a doctor whose professional interests and special preparation concern to the respective field. For example, an otorhinolaryngologist can change the form of nose or correct ears. But if he begins to operate the breasts or make liposuction, it will too further from his experience. The right preparation of plastic surgeon concludes the training during many years about the technology of the performance of the operations. But among qualified plastic surgeons also there is a huge difference in their experience of the performance of these or those operations. The patient must not be shy and has to ask the doctor about his qualification. It is necessary for the patient himself. There are four distinctions of good plastic surgeon: - careful training on the base of good foundation of the practical and theoretical knowledge. It concludes not only competence of this specialty, an official licence on practice of plastic surgery and base qualification but also an added training and experience of work in different methods; - high skill. There are thousands ways to do it. During the consultation with a plastic surgeon the patient has to receive the complete information how the operation will be performed. The patient often considers that there is only one way to perform this operation but truth concludes that the number of variants is almost endless. In the lifting of face, for example, the incision could be made by multiform ways. It may be performed in front of the ear or inside of the ear; an arch made low behind the ear or high in the fold behind the ear and upwards under hair. The skin around the incision may be separated weakly or strongly, sometimes only little on the cheeks or neck but sometimes to the end downwards to the fold being from the nose to the mouth, behind a central line on the neck and upwards to another ear. There are even operations are called "French lifting" or "minilifting" which is considered as a variant of lifting of the face. One makes a little incision in order to be able to lift only small piece of skin. The effect, of course, is not similar than in more large operation by which the superficial and also deeper structures are lifted. To prove scientifically, that this detail in this variant is better than that one, is very difficult because of that each surgeon finds his personal preferences on the basis of his experience and judgement. It is else more difficult for the patient to choose the variant of the operation. Some indications, however, one may receive listening a doctor with attention and, perhaps, collecting information from friends having an experience of the treatment by the means of plastic surgery. The private or public clinic. All of us know about financial problems of the state system, the medical insurance and its aspiration to cut the expenditures. Because of that last years the public clinics have to cut a budget radically. The cosmetic and aesthetic operations are performed in public clinics only in exclusive cases, perhaps, in frames of a research work or an educational course or very seldom in special psychiatric indications. The resources for such surgery for public money are restrictive and almost all patients with problems of aesthetic nature, as well as who have a desire to make a clear cosmetic operation, are addressed to the private practice. According to reconstructive surgery, there is else this surgery in some clinics of plastic surgery of the system of public health. There are private clinics and hospitals in all big cities, moreover, many of them appeared last decade. They are different about size and equipment - from small private practice only for the simple procedures to big registered hospitals. The small operations, as the removal of little spots on the skin, do not require a special equipment, enough very simple instruments. The large plastic surgery, which can last during many hours, requires already the complicated equipment and the set for the monitoring. Sometimes there is necessity to stay the patient in the clinic for a day or two days after the large operation, in many countries a special official permission is required for it. This certificate also defines the number of hospital beds and maximal number of the patients. How much does it cost? Why so expensive? All of us know that a price is defined by supply and demand, value of production and quality. Most of us do not wait for the highest quality and the lowest price at same time. As well as with plastic surgery. Although the operations are performed by the different ways and with a different level of nursing, the price of production is various. Perhaps, the operations in the Eastern Europe have much less production costs because the most part of the use equipment is worn-out morally and materially there. And the wage, besides, is low. Another example is an augmentation of the breast which many people consider a standard procedure but which can be different in the method and the duration and not only by a character of incisions but also the application of recently developing more progressive and expensive endoscopic surgery. The huge difference is also in the cost of implants. The most exclusive implants cost some times more expensive than the cheapest ones (a price for the couple from $ 400 to more $ 2000). Finally, a surgeon, who has large demand, costs, probably, more expensive than that one who has only some patients. The more popular doctors as lawyers may require the higher hour payment because demand on their services is high. According to it, to give the price-list of aesthetic operations is impossible. Thinking that most of these procedures last for one or two hours, it seems to us they are expensive. But one must remember that time, which was spent for the operation, is only part of all spent time. The preparation, postoperative treatment, next visits are also a lot of time. Besides, not only time of a surgeon has to be paid. Another personnel - surgical nurses, an anesthesiologist, an assistant, nurses must receive the wages. Besides, it is necessary to add the cost of equipment of the operative room or the cost of rent of the operative room which may be about some hundreds dollars per 1 hour.
