I hereby grant Farmak Joint-Stock Company (hereinafter referred to as Farmak) my irrevocable and unconditional written consent to process my Personal Data provided in the form I completed. Personal Data processing includes any actions and/or set of actions that involve gathering, registration, accumulation, storage, adaptation, alteration, update, use, and dissemination (distribution, sale, communication), depersonalization, and deletion of my Personal Data for the purposes of processing my inquiry or responding to my question.

Amount of my Personal Data which will be processed and may be included to Farmak’s personal data base is defined as any information about myself (provided in the form I completed) that became known to Farmak.

The consent I gave to processing of my Personal Data does not oblige Farmak to notify me about communication of my Personal Data to third parties under Article 21 of the Law of Ukraine On the Protection of Personal Data.
