Obtaining Civil Documents in Ukraine

To obtain copies of civil documents (including birth, marriage, divorce, death, and education records) on file in Ukraine, please submit a completed copy of the attached questionnaire to:

The Embassy of Ukraine will accept a request only if the information provided on the questionnaire is supplied in full and exact detail in English or Ukrainian.

The fee is $20 (twenty dollars) for a document search. It must be paid in advance and should accompany the search request; the fee will not be refunded if the document or record is not found. Therefore an applicant requesting a document should include the fee with the request in the form of a certified check or money order payable to the Embassy of Ukraine in Washington, DC.

6. Date (month, year) and reason of your departure from Ukraine

7. Kind of document wanted

8. When was original record made (day, month, year)
